Assessment and Support Planning Processes
HCBS Strategies works with states to build person-centered processes for identifying needs, goals, and preferences and incorporate this information in support plans that enhance control and increase community integration.
To efficiently and equitably develop person-centered plans that are goal-driven while minimizing health and safety concerns, states must develop processes that ensure the collection and use of accurate, reliable, and valid data about the person while finding the right balance of what is important for the person to maintain or improve their health and safety and what is important to the person to lead a meaningful life.
HCBS Strategies has consistently been a leader in developing person-centered support planning processes. HCBS Strategies worked closely with Minnesota State staff and stakeholders to develop the content for MnCHOICES, a comprehensive assessment and support planning processes that is used for all populations with disabilities. Since that time, we have led assessment and support planning reform efforts in Alaska, Colorado, Illinois, Maryland, and Oregon. These assessment processes target all populations with disabilities including older adults, individuals with intellectual and developmental disabilities, and medically fragile children.
HCBS Strategies is assisting states in taking assessment and support planning process to the next level through the ADvancing interRAI effort. Under this effort to build cross-state collaborative infrastructure to support the development and refinement of these processes. Under this effort, we are currently supporting seven states in refining their processes.
