HCBS Strategies works with states and local agencies to bring greater consistency, clarity, and accountability for staff that assist individuals in selecting and managing their supports. We have supported case management redesign efforts in Alaska, Colorado, and Hawaii.

Case management (such as care coordination, coaching, support coordination) is the heart of any LTSS delivery system. Case management is information sharing; case managers obtain information from participants about their goals and preferences. They also share information about providers and other supports that match these requirements. Case managers must employ effective tools and automation to support information sharing, while also establishing trusting relationships with participants. This dual approach ensures participants feel heard and supported throughout the process.

In addition to helping states with case management redesign efforts, HCBS Strategies incorporates our understanding of case management into the broader systems development work.  Our systems review assesses case managers’ qualifications, training, pay, and service requirements. We recognize that effective system improvement hinges on ensuring case managers have the resources to fulfill their responsibilities.

Our case management redesign efforts include reviewing and updating regulations, reimbursement approaches, automation support, training, and assessment, support planning and monitoring tools. We have trained case managers, included them in system pilots, and learned valuable lessons from them in focus groups and advisory committees.
