HCBS Strategies helped the Colorado Department of Health Care Policy and Financing (HCPF) beginning in 2014 on a variety of HCBS reform efforts, focusing on overhauling their assessment and support planning processes and redesigning their case management system.

The assessment and support planning redesign work has included:

  • Developing the content for the new processes 
  • Developing and managing a pilot of the content with over 700 program participants
  • Conducting analyses to replicate current level of care eligibility criteria
  • Developing program operations for implementing the new processes, including identifying optimal Medicaid authorities, changes to service requirements, and updates to policies and regulations
  • Collaborating with state training staff to design and manage trainings
  • Assisting state staff to oversee and test the automation of the new processes

All of this work has involved extensive collaboration with state staff and significant input from stakeholders.

The case management redesign work has included:

  • Strategic planning support to assist with designing the new structure to comply with Center for Medicare & Medicaid Services (CMS) mandates and meet other HCPF and stakeholder objectives
  • Conducting analyses and facilitating stakeholder input to finalize key redesign decisions, such as service catchment areas
  • Assisting with a complete overhaul of all HCBS waiver rules, including designing the structure of the new rules and supporting the stakeholder and formal review process
  • Supporting the implementation of local case management agency Community Advisory Committees (CAC)
  • Assisting with the development of policies and procedures for case management agencies
