HCBS Strategies has been supporting the Hawaii Executive Office on Aging (EOA) since 2010, acting in the role of the Systems Change Developer (SCD). This role has included:
- Working with the county-based Area Agencies on Aging (AAA) to develop a common Aging and Disability Resource Center (ADRC) approach consistent with guidance from the Administration for Community Living (ACL)
- Supporting EOA and the AAAs in implementing the new ADRC model within their county program operations
- Designing and supporting the implementation of a Veteran’s Directed Care (VDC) model funded by the US Veterans’ Administration
- Designing and supporting the implementation of the Community Living Program (CLP), a self-directed option under the state-funded Kupuna Care program
- Supporting a Person-Centered Hospital Discharge Planning pilot in Maui County
- Facilitating the development, implementation, and expansion of the ADRC effort into a No Wrong Door (NWD) network that includes private and public sector agencies representing all disabilities
- Facilitating the development of Medicaid Administrative Claiming (MAC) for ADRCs and NWD
- Assisting the state and AAAs in developing data-driven program operations and policy decisions.
